A sexually transmitted disease breaks out and affects several members of the hospital staff. Chief Webber promptly calls a meeting. Meanwhile, the chief reluctantly tells Dr. Derek Shepherd about his own medical concerns. Izzie and Cristina worry
about a patient's wife and daughter who can't agree on his care. Burke treats his college buddy Bill, whose medical diagnosis raises some questions regarding his wife's actions. Then just as Meredith and Derek grow more intimate, a surprise is revealed.
That's the problem with secrets --
Way to go, George. She's cute.
and I think I know what it is, but I can't get close enough to tell for sure.
To get a look inside the bladder.
Could be any number of things.
O'Malley, George. Look, it's just a simple blood test.
The only people who aren't like that are the amish and, apparently, you.
No, it's good advice, really good advice. Thank you very much.
And it all comes together.
I mean, not unusually long or anything, you know?
You dropped a retractor.
Dude, that's a good thing.
Oh, yeah, Alex? What's yours?
the condition can produce gonadal hermaphroditism.
Oh, get out. You're doing it wrong.
The reality is so much better.
Uh... sorry to bother you with this, chief.
When the time is right, and, gentlemen, you'll all know when that time is,
With every fresh banana, always use a fresh condom.
You don't need to be in this line.
There was blood in the tube when it first went in.
your husband dying so suddenly,
Alice, your father would have wanted --
Okay, Cristina Yang, licensed to kill.
Did you lock up the gallery?
When's the last time you did an autopsy?
You know how I know?
Holly, please! Tell him the truth!
- How advanced?
You could be arrested for assault!
The disease causes an excess amount of iron to build up in the body,
and when I realized, I broke it off with the other guy --

Meredith grows increasingly dissatisfied with the unknown details of Derek's life. Meanwhile, Alex treats Devo, a patient whose religious beliefs jeopardize her chances for recovery; Izzie contemplates reconnecting with her estranged mother; Zoey, a
pregnant woman in her 40s who's been diagnosed with cancer, disagrees with Cristina's medical advice; and the interns are mystified by Cable, a patient who's being treated for seizures and who thinks he's psychic.
You could give me your home number.
We were in the same frat at Tulane.
You don't know.
Maybe I've been sleeping around.
Maybe not quite like that.
It's always the quiet ones.
The swelling is pressing
against your lungs,
You better be.
I need to tell you something.
- A prostitute?
- No! Not a...
I'll set up some tests.
kind of like Superboy,
Everybody's got something to hide.
We have an excellent
gynecological surgeon on staff.
It's a shot of penicillin, George.
Hi. It's Ms. Henry again.
and, gentlemen, you'll all
know when that time is,
With every fresh banana,
always use a fresh condom.
Why are you in this line?
Mr. Franklin, we've given you
a local anesthetic,
Why do you think I want
to keep an eye on you?
[Woman over PA]
Code blue. Code blue.
But an autopsy would...
Oh, no, I'm not nervous.
[Laughing] I bet you do.
How's Burke gonna tell him
the baby's not his?
That's what killed him.
One gram, as ordered, sir.
You know what?
Hold on. Hold on. Wait.
You know how I know?
'Cause you're a bad liar!
If you were his friend,
you wouldn't do this.
She's a surgeon.
She's done this countless times.
- We want to run some tests.
- Oh, now you want to run tests?

Derek and Meredith try (unsuccessfully) to be discreet as they spend the night together at Meredith's house, but their haggard appearance speaks volumes to everyone - including Bailey, who warns Meredith to end the relationship. Meanwhile George
suspects an anesthesiologist of drinking on the job but is reluctant to speak up about it, Alex treats a former competitor who now finds honor in self-mutilation, Izzie has to deal with a patient who won't let go of his girlfriend - literally, and Cristina seems to be battling a case of the flu.
Next time, just let me know
if I need to go to a hotel
What's the problem?
but they called down
for a surgeon,
Excuse me, sir, uh,
you're bleeding. You mind if I...
My coach used to say,
it's all about how we process pain.
[Jamie] Mommy and Daddy.
On my way back to clinic.
I didn't give it to you.
It's all over the hospital.
It's not her fault,
I'll worry when he's too juiced
to do his crossword puzzle.
And a bad one, at that.
Tina, you know,
she tries so hard.
[Jamie] And nose?
I thought you would think
this is funny.
is a little more risky.
Yeah, I tried that, but...
Life doesn't work that way, Claire.
But, I mean, that's...
If the parents consent,
- [Lzzie] What, about doctor-cest?
- It's been going on for, like, ever.
You've lost some blood.
Needle-tip Bovie, please.
I've done everything
you've asked me to do.
Ah, coffee, where would
medicine be without it, huh?
[Taylor] Where do you get off
accusing me of something like that?
I wouldn't be here
if I wasn't.
Check that. I can't respect
a man desperate enough
Is he OK?
Is he gonna be OK?
It would qualify.
And now you smell like putrid goo.
I think Claire is killing
herself to please you.
His temp's way up,
and BP's dropping, doctor.
From the frontal lobectomy,
I'm going to encounter a, uh...
[Taylor] My bad, Shep.
Put the defib pads on him
and give him 150.
Dr. Yang, do you want to
add something to that?
Somebody should have
barring any complications.

Izzie tries to get George to find the courage to ask Meredith out. All of the interns are in awe when a woman comes in with a record breaking tumor, but it is so far advanced that the woman's prognosis is not good. Now that Bailey knows about
Meredith and Derek's affair, their jobs are in jeopardy, and she advises Meredith to end it. Meanwhile, Cristina and Burke's secret relationship is also getting more complicated...
I definitely have to end it.
Good morning, Annie.
How are you?
[Annie] I've been housebound
for the last year.
He actually seemed sincere.
It was running low
overnight down to 70l30,
But I don't want you
to say anything to her.
Not for Parkinson's,
for spinal pain.
There's going to be
a microphone in there for you
Well, what I don't understand...
She couldn't breathe.
Is that rude? I'm sorry.
I'm so tired.
You don't know what it's like
having a parent...
[Derek] If I miss a step,
- No. No, Annie.
I mean, thank you. OK.
if I don't ever ask?
How psyched are you?
if you drink alcohol,
I mean, we could
- Right here?
- Did you page?
Do I have to start drooling,
and forget my name
When those people start
finding out what's going on
Want to do it
in the stairwell?
We were able to relieve the
obstruction of his chest tube,
[Derek] This is going to sound
really scary, but try and relax.
It's as thick as a thumb.
You're doing great, Mr. Levangie.
Just a little longer.
[Derek] Bailey was on the warpath.
I was trying to protect you.
Does he have
a myocardial ischemia?
We have a pulse.
Oh, God. Just squeeze it off

Meredith realizes that she must take on a lot more responsibility when she is told that her mother needs to sign over all of her assets to her. As Izzie prepares to have a party for her boyfriend Hank (the scale of which is unknown to Meredith),
Meredith has her first heart surgery. During the surgery she fears that she has fallen asleep and damaged the patient's heart. Meanwhile, Cristina and Burke hit it off. At the party, Meredith and Derek get caught together by Dr. Bailey.
She must be having trouble breathing.
I'm allergic to aspirin, most NSAIDs.
Start a central line.
His veins are shot.
I've seen the films.
It's not all in your head.
Yeah, map has stayed around 80.
Cardiac output at five.
I think you'd probably know.
[Woman over PA] Dr. Shepherd
to the ER, stat.
We need to open her up.
I'm taking out the scope.
Best guess, her surgery five years ago.
Well, yeah. Yes, yeah, of course.
Yeah, great. OK, thanks.
No, ma'am, no. It doesn't.
Well, let me take you
out to dinner tonight.
BT, PTT, INR,
platelet counts were all stable.
- I'm sorry.
- You invited nurses?
- Did you clear this with Meredith?
Is there anything else I need
to bring beside my license?
The walls of her heart
are abnormally thin.
Oh, Jerry, it's not
you specifically. It's just...
OK, any luck yet?
You have a very annoying way
of sneaking up on people.
Don't tell me the notary didn't show.
You know, why did she
put this off for so long?
You have any bourbon?
He's blown his left pupil. Page
Shepherd. We got to get him to CT.
- More than he bargained for.
- [Alex] Maybe he's lucky.
I'm in my own house. My life is crap.
The hospital owns us. It's what we do.
This is my life now, Hank.
I work 100-hour weeks.
Apparently not good enough.
You missed Doctor-palooza.
I'm hoping it was yours.

While George is feeling a little less masculine because of the way Izzie and Meredith have been treating him (like a sister), Izzie is assigned to a patient about to loose all of his masculinity. Cristina, true to her competitive spirit, shows up
early and assigns herself to an ill woman, who happens to have been Meredith's moms scrub nurse. Alex, meanwhile, is busying himself copying and pasting pictures of Izzie's racy magazine spread all over the interns' locker room. And while Meredith is trying to cope with her mother's Alzheimer's, Derek treats a patient who has managed to get nails stuck in his head.
so you'd be the logical intern
for him to ask to scrub in.
If you think you're
gonna get lzzie to cover herself...
Use 4 milligrams of morphine.
Titrate up to 10.
You wife is on the way, Mr. Cruz.
Well, actually, Liz, I was gonna
give you to Meredith Grey.
CTs are down this morning.
I mean, some of the time, you know.
We won't know until the nails come out.
- Shepherd. Twenty-three cases?
- One was attempted suicide.
And she jumped out of the car
Oh, man. Look at those films!
and those who didn't.
- It's clean.
[Cristina] I wanted to know
if you've seen Fallon's labs.
[Man] Bye-bye, Liz.
Who is that?
She was excellent.
We have Bethany Whisper
in our locker room.
that put lzzie Stevens
through med school.
And on Saturday.

and allow me to do my job.
And they're gonna lift up my legs
and expose me to the world
I'm her cruise director,
pushing her around all day.
[Derek] There. That's a tumor.
I have to cut out a lot.
It's not funny, but...
[Lzzie] The woman's life
was this hospital.
It's makeup. It's retouching.
Dr. Hammond to OR. Room 3.
- [Man] I'll start ventilation.
- Start that line.
- I don't.

A bike race injures several pedestrians as well as well as participants. One pedestrian is left brain dead. Christina and Issie wish to use his organs for organ donation but with Christina's detachment and Issie's ever hopeful attitude that the
man will recover, they will find the organ harvesting process an emotional one. Preston faces feelings of his dislike of organ harvesting - as a surgeon, he likes saving lives; harvesting organs doesn't save the person's life. George deals with Mac, a friend of the chief, who won't stop flirting with him. He is in need of a new liver. Meredith is fed up with Alex showing off as she continues trying to push Derek away. She also is reaching the limits of Issie's sunshine and George's politeness as she goes on living with them. Then there is their desire to see Meredith's mother's medical tapes. Getting through the day, Meredith realizes the keys to winning the race.
- We have to do some tests
- Ow, wicked.
Uh, Dr. Shepherd?
He's not going to the OR?
A buddy of mine in 4451, Lloyd Mackie.
Give him whatever he needs.
Smoking will just
speed up the process.
He could wake up.
What about a miracle?
[Sighs] And, by the way,
you are a rockin' babe.
OK, well, you have to sign an AMA form.
- Get over yourself, already.
- Come on.
You know how many patients
we have downstairs.
There's this VIP patient...
So, you think you can
give that a shot for me?
We can't do anything.
It's not our place to make that call.
- He's gonna rupture and bleed out.
- He needs surgery.
- Not funny. This is important.
- O'Malley. How's Mackie?
The police called.
They've identified your John Doe.
[Mackie] You should reprimand him.
It's the smell of open-heart surgery.
Gotta get the cyclists
who were too drunk, stupid or scared
Are you willing to donate his corneas?
What are you doing?
I never thought you were gay.
[laughing] You know...
- [Nurse] Clear the way. Coming through.
- Somebody get the elevator.
Get me something to stand on.
Lower this table.
You're better at this part than me.
I'm gonna sew him up, for his family.

The surgical staff tries to be optimistic as a young woman clings to life after a brutal attack. Meanwhile Meredith puts her career on the line to save a newborn in the hospital nursery, and the other interns learn that it takes more than just medical knowledge to be good at their jobs.

She's breathing after what this guy did?
Oh, my God.

You know how slow they are.
She better take it with her.
Fit as a fiddle, or whatever.
- Oh, from the rape surgery?
- Yeah, and it's not a jar.
- [Meredith clears throat]
- [Pager beeping]
Don't people get sick anymore?

No. Both parents are dead.

Meet Meredith Grey. Daughter of famous surgeon Ellis Grey and currently recovering from a one-night stand. She shoos away her evening's companion, saying she'll be late for her first day of work. She is a first year intern at Seattle Grace Hospital
and is working alongside some colorful characters. Cristina Yang is a highly competitive Stanford graduate who befriends Meredith. George O'Malley is a quirky warm hearted man who learns a lesson or two on his first day. Izzie Stevens is a former model who doesn't get any special treatment on her first day. And Alex Karev is an arrogant jerk who misdiagnoses a patient on his first shift. Overseeing them is Miranda Bailey, (not-so-) affectionately dubbed 'The Nazi'. Meredith also gets a surprise when she realizes that her friend from the night before is a co-worker, and even worse, her boss.
Uh, we met at the mixer. You had on a black dress with a slit up the side, strappy sandals...
Rule number one -- don't bother sucking up. I already hate you. That's not gonna change.
You said five rules. That was only four.
A large-bore I.V. Don't let the blood hemolyze. Let's go!
I wish I could, but I'm a healer.
The nazi's a resident. I have attendings hating me.
It's posted, but I thought I'd share the good news personally.
I get it, I respect it, but George O'malley's a puppy.
I was drunk, vulnerable, and good-looking, and you took advantage.
He's gonna faint. He's a fainter.
Today. Pull your balalls out of your back pocket. Let's go.
How many times do we have to go through this, George? 5, 10?
Can you call someone?
Well, you know what that means.
Next time you wake me, he better be so close to dead, there's a tag on his toe.
- You paged Dr. Shepherd?
- I just told you.
I don't have an answer for you.
There are some risks.
I don't care.
I know you're tired. You're busy. You got more work than you can possibly handle.
You can't comment, make a face, or react in any way.
She's just never gonna get the chance to turn into a person.
She has no headaches, no neck pain, her C.T. is clean.
Love to stay and kiss your asses, but I got to tell Katie's parents she's having surgery.
You never promise a patient's family a good outcome!
Don't come to me for absolution. You want to be a shark, be a shark.
You shouldn't let the fact that we had sex get in the way of you taking your shot.
- Uh, yes.
- From your head, not from a book.
